Emergency Preparedness Survey for Residents

As local emergency and disaster planners and responders we would like to know if our community residents have taken the first steps to being prepared for an emergency or disaster.
Your answers will be anonymous. The information will be tallied by Columbus Public Health and used by professional planners and responders to prepare and respond to emergencies and disasters locally.
The information from this survey will help align programs and resources to the right areas in our community.
