School-Based Vaccine
Clinics are set to begin for the 2017-2018 school year. Our next set of clinics
will be to provide influenza (flu) vaccines at select Columbus City Schools in
late fall, 2016. Please contact your school nurse to find out when Columbus
Public Health is going to your school!
If you have not had your child vaccinated with their required shots
before the next school year, you can visit our Immunization Clinic.
Columbus Public Health’s Immunization clinic
will have EXTENDED HOURS during Exclusion Week to ensure your child can remain
in school or return to school as quickly as possible:
Saturday, September 9: am-12pm
Monday, September 11: 8am-2:15pm
Tuesday, September 12: 8am-7pm
Wednesday, September 13: 8am-7pm
Thursday, September 14: 8am-4:15pm
Friday, September 15: 8:15am-4:15pm
Saturday, September 16: 8am-12pm
You
can easily sign your child up now to receive school-based vaccines (flu, Tdap
and Meningococcal), as well as other important health services, by completing
the new School-Based Supplemental Health
Services Consent Form and returning it to your school nurse. This completed consent form will cover your
child for the entire 2016-2017 school year.
