Chlorination Request Form and Checklist
Projects with water mains that are ready to be disinfected will need to submit a Chlorination Request Form. The Request shall come from the inspection agency for the project and be submitted electronically to the Division of Water. The Request Checklist should be useful in this process by listing all aspects required in the request form, pressure test reports, as-built plan sheets, and water service reports.
