To help prevent the spread of illness, walk-in services for birth and death records will not be available at Columbus Public Health beginning March 16, 2020, until further notice.

Death certificates can be registered online or by email. Please email all new paper death certificates to

24-Hour Service

Burial Permits are available to funeral directors 24 hours a day, 7 days a week at the following locations:

  • Vital Statistics Office (during regular business hours)
  • Sub registrar or local Vital Statistics Registrar (it is recommended you contact the appropriate party first for availability prior to approval)
