Columbus Public Health serves all people with fairness and equity and provides services in a way that is just and free from bias or prejudice.


Fair and respectful service is your right. You have the right to:

  • Be served with fairness, equity, respect and dignity.
  • Not be discriminated against.
  • Receive information in a way that is easy to understand.
  • Personal privacy.
  • Be referred elsewhere for related services Columbus Public Health does not provide.


Language help is your right. You have the right to:

  • Request and receive language services at no cost to you.
  • Request and receive documents in your language.
  • Request and receive auxiliary aids and documents in accessible formats.
  • Request and receive an interpreter.

Please let staff know you need language help or use an ‘I Speak’ card at any front desk area.

Giving feedback is your right. If you feel you have been mistreated or felt unsafe, it is not your fault. You can file a complaint:

  • Talk directly with health department staff.
  • Email CPHCustomerService@columbus.gov.
  • Call 614-645-6972 or 3-1-1.
  • Mail to Columbus Public Health, Attn: Office of Planning & Improvement, 240 Parsons Ave., Columbus, OH, 43215.
  • Fill out a feedback form online at https://redcap.columbus.gov/surveys/?s=JDMHW83THT343C3C.
