(Single Vehicle or Multiple Vehicles) Be sure to include an email address to receive a copy of your report once it has been processed. If you are unsure of the answer to any of the required fields, select or enter "Unknown." Any crash report filed this way will not receive any investigative follow-up and is only for documentation and insurance purposes.
