• Insurance - Insurance cards must be presented at time of service, and will be accepted if the patient is eligible for coverage. To use an insurance card you must present photo ID.
• No child 18 years or younger is refused for inability to pay

All routine children's vaccines are available at Columbus Public Health. Some immunizations for high-risk conditions may require a prescription from a physician. Columbus Public Health does not provide travel immunization.

Recommended Children's Vaccines:

• Hepatitis A and B
• Tetanus, Diphtheria, and Whooping Cough (Tdap)
• Polio (inactivated)
• Pneumonia (Prevnar-13)
• Haemophilus Influenzae B (Hib)
• Rotavirus (Rotateq)
• Meningitis (MCV4)
• Measles, Mumps, and Rubella (MMR)
• Chickenpox (Varicella)
• Human Papillomavirus* (HPV4, Gardasil)
• Influenza (during flu season)

Children 18 and under may receive all required vaccines, and if eligible, all recommended vaccines.

Adults with No Insurance:

Subject to the availability of vaccines, adults 19 years of age or older may receive all routinely recommended adult vaccines (except Shingrix) for a zero vaccine cost. However, a $16.00 assessment fee will be charged per visit. A sliding fee scale is available and no adult who qualifies will be turned away for inability to pay.

Adults with Health Insurance:

• Only the flu vaccine is available for adults with any form of health insurance.
• Adults who carry any form of health insurance are advised to go to their provider or check availability of vaccines at any local pharmacy.

What Vaccines Do You Need?

More Details:

• Columbus Public Health does not provide travel immunization.
• For a full list of recommended adult vaccines, refer to the the CDC vaccination schedule for adults.
• Columbus Public Health strictly adheres to the vaccine schedules established by the CDC's Advisory Committee on Immunization Practices (ACIP), and does not provide immunizations off label.

How to Obtain Copies of Immunization (Vaccine) Records:

• Contact the healthcare provider where you got your most recent immunization
• Contact the school where you most recently attended
• Columbus Public Health can provide a copy of a record only if the vaccines were given at Columbus Public Health

To obtain a copy of your vaccine record for vaccines that were given at Columbus Public Health, please call (614) 645-2738.

**Please note that the Health Department staff are not able to tell you over the phone whether or not you have a vaccination record in the system. You must call the Medical Records number above.

Flu vaccines are offered in many locations including doctor’s offices, clinics, health departments, pharmacies, and college health centers, as well as many employers and some schools.

Local Flu Vaccine Sites for Children and Adults:

• Check with your pediatrician or family doctor about flu shots.
• Check with your local retail pharmacy, most offer flu shots for adults and some offer flu shots for children of certain ages.

Prevent getting and spreading the flu…wash your hands, cover your cough, stay home when sick, and get your flu shot every year!

At this time, Columbus Public Health does not have COVID-19 vaccines for adults. We encourage you to visit your health care provider or local pharmacy to get your COVID-19 vaccine.
