Tester's documents and forms (PDF files):

• Credentials Expiration Information(PDF, 623KB) – for Device Testers Updated 11/21/25 (typically updated biweekly).
  List of the credential expiration dates on file with the Backflow Compliance Office (BCO) that are used to determine a tester’s approved/unapproved status. Testers are responsible for providing the BCO with up to date copies of their credentials.
