Location: Connectors will work primarily out of the Moms2B sites at New Salem Baptist Church (2956 Cleveland Avenue), Grace Missionary Baptist Church (1182 North 6th Street), Hillcrest Baptist Church (2480 West Broad Street) and Mount Carmel West Healthy Living Center (777 West State Street). The main office is located at 921 Chatham Lane, Suite 108 Columbus, Ohio 43221.

Tentative Working Hours and Hours per Week: Approximately 25 hours/week. Working hours will be determined based on individual availability and the needs of the program. Weekend and evening hours will be required. Attendance at community events that occur in the evenings and/or weekends, including but not limited to health fairs, baby showers, festivals, is expected.

Hourly Rate: $13/hour

Preferred Qualifications/Skills:
Basic computer skills, including emailing. Ability to initiate and maintain a positive working relationship with Moms2B team members and Moms we serve. Great communication skills and ability to follow up with Moms. Ability and willingness to provide support and encouragement to Moms. Ability to multi-task and quickly adjust to needs of the program. Experience working in a community-based setting is preferred. Previous work experience is preferred, although not required.

Transportation Requirements: Connectors must have reliable, consistent access to personal transportation. Frequent travel, within Columbus, is required and expected.

Job Description: Connector provides information, support and resources to moms and their babies enrolled in the Moms2B community-based program. Connector will act as a liaison between community service providers and program providers and among Moms2B program professionals to facilitate accurate and prompt implementation of program participants. Connector will coordinate collection of information for client care documentation and maintain client chart information. Connector will contact patients before, beginning and during Moms2B programs to address questions and identify barriers or concerns that may interfere with their participation in the program. Connector works with managed care organizations to ensure minimal financial burden on clients and collaborates with social workers and other team members to facilitate client participation. Connector will participate with other team members in quality assessment and improvement activities and identify opportunities for improvement in client care and interaction. Connector will be responsible to attend weekend community engagement opportunities that may include health fairs, baby showers and more.
