Community Development for All People (CD4AP) is seeking engaging, professional individuals for a dynamic position serving pregnant women, new parents and their families living within zip codes 43205, 06, and 07. Community Health Workers will provide leadership to efforts aimed at positively impacting issues of infant mortality here on the South side.

Tentative Working Hours and Hours per Week: 25-30, primarily Monday – Friday with some evening and Saturday events

Preferred Qualifications/Skills: Able to provide own transportation; computer literacy especially using MS Office programs and Google drive documents; ability to build rapport; professional attitude; ability to be flexible,  

Hourly Rate: $14/hour

Benefits Offered: Paid holidays and paid time off as a percentage of hours worked.

Transportation Requirements: Must be able to provide own transportation to meet with families in different settings. Most visits are not on a bus line, therefore, having a drivers’ license and access to a reliable car are essential.

Job Description: 
Overview
Community health workers (CHWs) are frontline workers with a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. 
Responsibilities
     •     Help individuals, families, groups and communities develop their capacity and access to resources, including health insurance, food, housing, quality care and health information
     •     Facilitate communication and client empowerment in interactions with health care/social service systems
     •     Help health care and social service systems become culturally relevant and responsive to their service population
     •     Help to build understanding and social capital to support healthier behaviors and lifestyle choices
     •     Deliver health information using culturally appropriate terms and concepts
     •     Link people to health care/social service resources
     •     Provide case management, support and follow-up
     •     Advocate for local health needs
     •     Make home visits to pregnant women and nursing mothers, and 1st Birthday moms
     •     Provide information to the general public through tabling and event participation
     •     Conduct intake interviews and facilitate enrollment of pregnant mothers and mothers of children under the age of 1 into the 1st Birthday program
     •     Share the safe sleep message wherever and whenever possible as a Safe Sleep Ambassador
     •     Visit social service providers to determine eligibility requirements, referral practices and accessibility (fact finding)
     •     Document interactions with clients utilizing organization’s software/file protocol
     •     Attend staff meetings, case review sessions, team meetings etc. as requested
     •     Other duties as assigned
Minimum Qualifications
     •     High School diploma or the equivalent
     •     Successful completion of a Community Health Worker formal training program such as from a college or other educational institution
     •     Experience working in a diverse setting
     •     Basic computer skills
     •     Good communication skills and ability to build rapport
     •     Must have reliable vehicle and transportation
     •     Familiarity with Microsoft Office Suite
