Tester's documents and forms (PDF files):

• Testers Application Form for Certified Backflow Assembly Testers; return this completed form to Backflow Compliance Office Updated 1/10/23
• Testers Registration Payment Form to be included with payment of your new tester and/or annual renewal fee; send to Revenue Reception Office. Updated 1/22/24
• Credentials Expiration Information – for Device Testers Updated 4/24 it is typically updated bi-weekly.       
  List of the credential expiration dates on file with the Backflow Compliance Office (BCO) that are used to determine a tester’s approved/unapproved status. Testers are responsible for providing the BCO with up to date copies of their credentials.
